Name of Game: The name you want the event to be listed as.
GM: Name of person running it.
Type of Game: RPG/CCG/Board Game/Traditional/LARP/Card Game
Estimated Game Length (min/max): Least amount of time you think you'll need and the most amount of time you think you will need.
Number of Players (min/max): Minimum amount of players needed to play and max you can have playing.
Preferred Date/Time to Run Game: On which day and at what time you want to run your game, can put multiple times you want. If you want to run the game several times at different times that's fine.
Description: Description of the game and how it will be played.
Game, System, Rules: Actual name of the game, RPG system, or what rule set you are using.
Game Complexity: Easy, Basic, Detailed, Complex
Experience Required: None, Some, Moderate, Expert, Extensive
Materials Provided or Suggested: Yes, and/or list what the players need to bring or are suggested to bring to make things easier.
Space Needed: How much table space you think you'll need.
Tournament: Yes or no
Tournament style: elimination, swiss, or what
Prizes Awarded: yes or no, this refers to if you or a sponsor is putting up the prize
Game Sponsors: Do you have anyone sponsoring you? Like a club, a company, or anything like that.
